Submission Requirements:
- Completed ACORD Applications, signed by the insured
- Program Supplemental Questionnaire
- RecycleGuard Program Supplemental Application(s):
- Auto Dismantler Supplemental
- Rubber Supplemental
- Metal Supplemental
- Paper Supplemental
- Plastics Supplemental
- Pollution Legal Liability
- Workers Compensation (provided by another carrier)
- 5 years of recently valued company loss runs with details on losses over $10,000
- Full description of operations, including those of additional Named Insureds
- Drivers list, including MVR's
- Complete vehicle identification numbers (VIN)
Broker Requirements:
Current appointment with the program administrator or, if not, complete the following items:
- Broker Agreement
- IRS W-9
- Copy of E&O Certificate or Declaration Page
- Copy of the agency’s license per location
- Agency Profile Document
- If multiple locations, include a list and confirm ALL locations are covered under the operation’s Errors and Omissions policy
