Submission Requirements:

• For sporting related contests, application available online through the program administrator’s web site.
• Prize Indemnity application available for offline quoting

Broker Requirements:

• Program administrator appointment with broker agreement, W-9, questionnaire, and e-signature form for online system
• Licensed Agent / Broker with current E&O limits of at least $1 million
